Court Order; Order of Administrative Tribunal - WHFPT will disclose protected health information in accordance with an order of a court or of an administrative tribunal of a government agency.

Coroner - WHFPT will disclose protected health information to a coroner where the coroner requests the information to identify a decedent; to notify next of kin; or to investigate deaths that may involve public health concerns, suspicious circumstances, child/elder abuse, or organ or tissue donation.

Child/ Elder Abuse Reporting - WHFPT will disclose protected health information about a youth or member of his/her family who is suspected to be the victim of child/elder abuse to the extent necessary to complete any oral or written report mandated by law. Under certain circumstances, WHFPT may disclose further protected health information about the youth and/or his/her family to aid the investigating agency in performing its duties. WHFPT will promptly inform the youth or his/her parent or guardian about any disclosure unless WHFPT believes that informing the youth or his/her parent or guardian would place the youth in danger of serious harm, or believes informing his/her parent or guardian would not be in the youth’s best interest.

Other Disclosures Required by Law - WHFPT will disclose protected health information about a youth and/or his/her family when otherwise required by law.
Kids and Privacy
For children who visit our site, special rules apply. We do not request personal information about children, such as first and last name or street address and city. When kids send email to us, their online contact information (email address) is not used to re-contact them and is not maintained in retrievable form.
Opt-Out or Change Your Contact Information
The WHFPT site provides users the opportunity to opt-out of receiving communications from us through a special online form. You may also update your contact information previously provided to us through another online form.
Third Party Services
This Privacy Policy does not address, and we are not responsible for, the privacy, information or other practices of any third parties, including any third party operating any site or service to which Services link.
